5.2 Weeks 1-3: Initial consultation and early monitoring
In preparing for the consultation, if the person has their repeat prescriptions dispensed at your pharmacy, you can use your clinical records to get an indication of current health conditions.
Having a structure for the consultation helps to ensure that the key issues are covered. An example included in Table 1 and is loosely based on the ASK, ASSESS, ADVISE, AGREE, ASSIST framework.

If you are using a software platform to structure and record your consultations, the suggestions in Table 1 on the next page can help you with question wording and probes as well as signposting to other tools. Individual pharmacists will find and become familiar with ordering and wording that works for them.
