Warning for superintendent after medicines supplied ‘without valid prescriptions’

The General Pharmaceutical Council’s investigating committee (IC) has issued a warning to a pharmacist after “serious” failures were uncovered at a pharmacy where she acted as superintendent.
Adriana Slima worked as superintendent pharmacist at the pharmacy in question between January and May of 2024.
A GPhC inspection during that period identified “serious governance and systems failures,” including the supply of medicines without valid prescriptions, inadequate record keeping and medicines “compounded without appropriate supervision and without ingredients sourced from approved UK supplier”.
The regulator placed restrictive conditions on the pharmacy following the inspection.
Ms Slana’s attendance at the pharmacy was described as “limited”.
The IC found that although she had raised concerns with management, she “failed to take further appropriate action when those concerns were not addressed”.
“This demonstrated a lack of sufficient insight expected of a superintendent pharmacist,” said the committee.
She was found to have breached professional standards relating to the use of professional judgment, speaking up when things go wrong and demonstrating leadership.
Ms Slana was warned that she must ensure effective governance arrangements are in place and “actively assure herself” that a pharmacy is operating in a safe and lawful manner.
The warning will appear on her register entry until June 24, 2027.
