Personal action plan for diabetes sufferers

A new scheme will see people who do not have their diabetes under control being given personal action plans.

Under the scheme, launched by Diabetes UK, the NHS and private providers, when a patient is not meeting their targets for blood pressure, cholesterol or blood glucose, an alert will flash up on their GP's computer advising them that their patient could benefit from an €information prescription€.

The one-page document is tailored to each patient and will contain information on how to better manage their health, such as joining a walking group or testing blood glucose levels more often. Only 36 per cent of people with diabetes are currently meeting all three targets.
